The Medical Release Form is to be used by coaches and team managers to ensure that they have a parent's permission to call for medical attention for a child on their team roster if the parent isn't present, and that the parent will cover the costs of the medical treatment provided. It should be kept in the team folder, and provided to any First Responders if requested.
